Care Alerts electronically inform physicians and physician extenders when one of their patients seeks care at an emergency room or is admitted to or discharged from a hospital. Care Alerts promote immediate follow-up and transition of care coordination, which reduces the likelihood of a hospital readmission. Providers will receive alerts for patients assigned to them. Patients are assigned to providers whom they have seen most often.
Beginning January 1, 2020, enrollment in Care Alerts will be required for participation in the BCBS of AL Primary Care Select Program or OB-GYN Select Program.
ProviderAccess administrators can enroll by following these steps:
- Log in to ProviderAccess.
- Click “Users” in the upper-right corner of the screen.
- Select the tab labeled “Care Alerts.”
